What happens next?

1. Read the CNM manual for the requirements to apply. You can view or download the manual here Forms & Manual Page
2. Choose if you wish to apply online via the link provided or manually. Prior to applying use the manual to gather the required documents including proof of CSNM membership and 16 CE points.

4. For manual applications please submit your application package and payment* to:

* Your payment will not be taken unless your certification has been approved. For online applications you will be send a payment link once your application is approved.
To submit your applications, we accept online, mailed copies, faxes, and emails with scanned PDF attachments.
